Client Code of Conduct
Clients agree by scheduling services to adhere to the following:

they have put every medical condition they have on the intake forms

they are truthful about whether or not they have consumed alcohol on the same day as their appointment.

they have the accurate medication that they are currently taking on file.
-
they are truthful about prior skin treatments/frequency of prior skin treatments.

they inform their practitioner of any doctor's recommendations.
-
they will be charged for missed appointments based on our cancelation policy.
-
they will adhere to safety prompts from practitioners when given.
-
they will adhere to treatment adjustments based on licensed knowledge.
-
they will not discriminate or behave in a manner where they become a danger to themselves/others

Clients agree when on Eye of Illumination property to adhere to the policies and instructions of the staff of Eye of Illumination as listed in client conduct.
At anytime if Eye of Illumination policies are invalidated by client conduct, Eye of Illumination holds 0 liability for client's loss of service, loss of payment, and/or reports mandated by State & Federal laws.


Eye of Illumination and all our businesses encourage people to follow our social media pages, however, any behavior which violates the law and/or causes harm will cause an immediate ban from all Eye of Illumination social media accounts.
